Progress in solid organ transplantation that has occurred over the last two decades is mostly driven by the improvements in short-term outcomes. Such improvements have been mostly related to the development of effective immunosuppressive drugs for the  prevention of acute rejection. In fact, prevention of acute rejection has been the principal aim in transplantation medicine for several decades, therefore most of the clinical trials in solid organ transplantation have been performed in the first year or two after transplantation.
