The registration fee is € 122 (VAT included) and includes meeting attendance,
lunch and light refreshments on December 6th 2017.
How to send form and payment
- Delegates can pre-register until 5th December 2017.
- Download ad fill in the pre-registration form.
Please send by email the pre-Registration form to firstname.lastname@example.org
- Do not forget to send the Pre-Registration form completely filled in, when you send the payment by bank transfer or by Paypal:
without it we cannot insert your name in the pre-registered delegates’ list.
- Payment of fee and form must be submitted at the same time.
- Name of participant must be specified in payment description.

Congress material can be picked up at the Registration Area of Starhotels Du Parc, starting December 6th at 7.30 am.
Certificate of attendance / UEMS / ECM
The certificate of attendance and UEMS will be sent to all regularly registered delegates by email after the course.
In case you do not receive the certificate of attendance by email, please write to firstname.lastname@example.org
On-site registrations will be possible starting from December 6th at 7.30 am at the Registration Area of Starhotels Du Parc, viale Piacenza 12/C- 43126-Parma.
Information such as name, full address, e-mail and phone will be mandatory for each registration, whether personal or sponsored.
It will be possible to pay cash (EUR only), no credit card payment will be possible.
- Within November 30, 2017- 100% refund.
- From December 1, 2017- no refund.All refunds, if any, will be done after the Meeting and will be in EUR currency only.
